This diabetes model of care includes an ATSI virtual team, GP-led diabetes care, QAS pathway and home monitoring.
The model has assisted the service delivery and integration of services, increased capacity of GP-coordinated multidisciplinary services, developed an efficient interface, improved accessibility and effectiveness.
