This new facility uses the latest technology to provide real-time data from multiple sources that a dedicated team uses to improve the flow and care of patients.
Opening in November 2017, the Coordination Hub has been uniquely designed to coordinate and navigate the patient journey through the acute care setting and support them back into the community setting.
