REMODeL is an innovative care model for people with type 2 diabetes that challenges the concept of ‘routine’ clinic visits to the specialist diabetes service through remote monitoring, automated tailored text-based feedback and periodic online patient self-report. The study showed there were significant improvements in glycaemic control, was highly acceptable to patients, and this was achieved with fewer in-person visits.
