CPCs are located on the CPC website and are embedded in Healthpathways.
The first contact many clinicians in primary care will have with CPC will be as part of making a referral using HealthPathways.
HealthPathways is a web-based portal with evidence-based information on the assessment and management of common clinical conditions including referral guidance. The pathways are written by General Practitioners with support from local GPs, hospital based specialists and other subject matter experts.
Clinical Excellence Queensland is working with Hospital and Health Services and Primary Health Networks across Queensland to implement.
HealthPathways are live across Queensland. Pathways are being added regularly across all local sites.
The CPC program team continues to work closely with the HealthPathways Coordinators across the state and with other services to ensure that the pathways contain the most up-to-date information.
